Local contractor fined after OSHA investigation

Bettendorf-based Hawkeye Paving was fined $4,000 after a construction site accident that killed a Davenport man in 2012.

Bettendorf-based Hawkeye Paving was fined $4,000 after a construction site accident that killed a Davenport man in December 2012.

Thomas McLachlan, 56, died when the bulldozer he was operating overturned.  Police said the ground gave way and McLachlan was trapped beneath the overturned bulldozer.

OSHA investigators concluded Hawkeye Paving did not do regular inspections to ensure heavy equipment operators were wearing their seat belts as a safety precaution.
